Journey Martial Arts Child Watch Waiver & Release of Liability
Terms & Agreement
I attest that I have fully read this legally binding document in its entirety. I, the undersigned parent or legal guardian, acknowledge that I am voluntarily enrolling my child(ren) in the Journey Martial Arts Child Watch Program and agree to the following terms:
1. Supervision & Responsibilities
- The Child Watch Program is a courtesy service provided for parents participating in classes and for siblings of existing Journey Martial Arts students that are actively participating in a class.
- I understand that this is not a licensed daycare and staff are not required to be certified child care providers.
- Child watch staff will supervise children but are not responsible for feedings of any kind, diaper changing, or administering medications.
2. Health & Safety
- I agree that every time my child is dropped off for the Child Watch Program, I have confirmed, to the best of my knowledge at the time of each drop off, my child is free from illness (fever, vomiting, contagious conditions) and can safely participate in child watch activities.
- If my child becomes ill or distressed, I agree to return immediately to tend to their needs.
- I agree that I am not allowed to leave the premises while my child is in the child watch center.
- In case of an emergency, I authorize Journey Martial Arts staff to administer first aid and/or contact emergency medical services if necessary.
3. Behavior & Conduct
- My child must be respectful and non-disruptive while in the child watch area. If behavior becomes unsafe or disruptive as determined by Journey Martial Arts Child Watch worker, I understand that I may be asked to remove my child.
- I acknowledge that Journey Martial Arts is not responsible for lost, stolen, or damaged personal items.
4. Liability Waiver
I hereby release and hold harmless Journey Martial Arts LLC, its owners, staff, independent contractors and affiliates from any and all liability for injuries, accidents, or illnesses that may occur while my child is in the child watch program. I fully assume all risks associated with my child’s participation.
5. Acknowledgment & Agreement
I have read and understand this waiver in its entirety. I agree to abide by the rules and policies of the Journey Martial Arts Child Watch Program. By failing to abide by the rules and policies, I understand that my child will no longer be allowed to participate in the Journey Martial Arts Child Watch Program.
